Insurance
We accept and bill most insurance plans. Please bring in a copy of your insurance card when you come for your visit. Often, even if we are not a preferred provider for your plan, you are still covered in our office, but your percentage of personal balance may be slightly higher.
At this time, we are not accepting the Oregon Health Plan. If you have a question about whether your plan can be billed in our office, please feel free to call us at (541) 451-1176.
Payment Options
Payment is expected at the time of service. Insurance copays and deductibles are estimated and patients pay their portions at the time of service. We accept Mastercard, Visa, and Discover credit cards, as well as cash and personal checks. A five percent cash discount is offered if treatment is completely paid for on the day of service.
